Congratulations on deciding to enroll as a representative with Community Assistance Program. Complete the application below and we will contact you shortly.

You will need the following information before we get started:

• Your contact information.
• Credit/Debit card for enrollment fee. If you do not have a credit card contact your sponsor to continue with online enrollment.
• Bank information for commission payment.

You will need to read and accept our contract and policies and procedures. Complete the process by filling out the sign up form.
